PSW Medical Check / Health Report

The Medical Report outlines the mandatory medical requirements of the program including physical ability and required immunizations.

Directions

  1. Download the appropriate Medical Check/Health Report Form

    • PSW DE 2022 - Pre-Placement Health Form

    • Note: We are currently using “Placement Pass” and are submitting the documents directly to the Placement Facilities. Therefore, ignore any steps involving uploading to “Placement Pass”

  2. Review the requirements checklist on the form:

    • Section A: The Medical Requirements Include: 

      • Tuberculosis Screening

      • Measles, Mumps and Rubella (MMR)

      • Varicella (Chicken Pox)

      • Tetanus/Diptheria (Td)

      • Pertussis

      • Polio

      • Hepatitis B

      • Influenza

      • COVID-19 (2-doses)

    • Section B - Non Medical Requirements

    • Standard First Aid and CPR (Level C)

      • Can be completed at Metrocare, for upcoming dates: click here

    • N95 Mask Fit Certificate

      • Can be completed at Metrocare, to purchase: click here

    • Vulnerable Sector Police Check

      • Due: Within 45 days of Class Start 

    • WHMIS

      • Completed with the Clinical Placement

    • AODA Certificate 

      • Completed with the Clinical Placement

  3. Bring vaccine records, public health forms or documents that show your immunization history to your appointment.

  4. Provide Section A (instructions and forms) to your health care provider to complete, initial, and sign/stamp. Note: RNs/RPNs may also co-sign portions of the form.

  5. Ensure your health care provider gives you the following documents:

    1. Vaccine records (with name on each record), including childhood immunization records if available.

    2. Laboratory blood results

    3. Chest X-ray report, if required.

  6. Complete Section B: Mandatory non-medical requirements and provide certificates or proof of completion for each requirement.

  7. Complete checklist (above) to ensure all requirements are met for both sections (A & B).

  8. Scan, label, and submit all documents to support@metrocarehealth.ca or in-person

Cost

  • Fees depend on your OHIP coverage and physician’s fees

Deadline

  • Within 45 days of Class Start AND valid within 6 months of Placement
